I've spent years interviewing the top doctors in the menopause space. I've read the research, lived it, talked about it on this show more times than I can count. And still, sitting across from Dr. Jason Fung, he reframed something I thought I already understood. The belly fat conversation isn't about food. It's about hormones and here's what that actually means.
1. By the time you're pre-diabetic, insulin resistance has already been happening for years.
Most women in perimenopause are never tested for insulin resistance until they're already pre-diabetic. But insulin resistance starts much earlier than that. Estrogen plays a protective role in how your body processes insulin, and when it drops, that protection goes with it. Which means the belly fat isn't just cosmetic, it's a signal that your metabolic health is shifting, often years before any doctor flags it.
2. The hunger you're fighting isn't always physical.
Dr. Fung breaks hunger into three types and only one of them is your stomach growling. The second is hedonic hunger, your brain seeking pleasure or relief through food. The third is conditioned hunger, which is essentially Pavlovian. You've paired eating with so many triggers over a lifetime that your body now generates hunger on cue: sitting down to watch TV, walking through an airport, the smell of a coffee shop. Women in menopause are already dealing with disrupted sleep and elevated cortisol, both of which amplify hedonic hunger significantly, so the cravings aren't weakness, they're a stress response.
3. Fasting works differently in menopause and most women are either overdoing it or dismissing it entirely.
Dr. Fung's actual recommendation is quieter than you'd expect, a consistent overnight window every single day without even calling it fasting. What that window does is allow insulin levels to drop low enough that your body can access stored fat for energy, something that becomes harder in menopause because insulin stays elevated longer. The longer you wait to give your body that window, the harder the metabolic math gets.
