For too long, testosterone has been miscategorized as the “male hormone.” But as Dr. Kelly Casperson revealed on The Tamsen Show Podcast, testosterone is actually the dominant sex hormone in women and it's essential for more than just libido.
“It is wrong when people say estrogen is our main hormone,” Dr. Casperson shared. “Our main hormone is testosterone.”
This neurosteroid plays a critical role in brain health, muscle mass, mood, and yes, sexual desire. And here’s a surprising truth: estrogen is made from testosterone. So when your levels drop, particularly in perimenopause, you feel it everywhere from your energy, to your gym results, to your motivation, and even your ability to connect intimately.
But there’s a catch. In the U.S., there is still no FDA-approved testosterone product for women, leaving many to turn to off-label options like compounded creams, microdosed male gels, or high-cost pellet implants. Four other countries have already approved female-specific testosterone formulations, and the U.S. is expected to follow.
Dr. Casperson emphasized the need for individualized care and testing preferably using a mass spectrometry test sensitive to female levels. But she also reminded us that “there is no perfect number that says who qualifies and who doesn’t. It’s about how you feel.”
